Robin Run 5K and Family Fun Day 2024 - Parental Consent Form

Parental consent is required by Nottingham Hospitals Charity for the Robin Run 5K and Family Fun Day event for any 16 year olds and under in your party.

I agree to the above child/children to take part in the Robin Run 5K and Family Fun Day at Sherwood Pines on Sunday 19^th May and have read the information about the activity and the event.

Although every precaution will be taken to minimise risk, the run can be a strenuous activity and the walk can pose minor hazards (eg trip/slip hazards) and both can pose a risk of personal injury or unlikely death. By signing this form you are confirming that you have read and agree that:

•  You (and/or your child/children) are aware that the 5K run can be potentially strenuous and the walk has minor hazards
• You (and/or your child/children) are not aware of any medical or other reason that you should not participate
• Your child/children will listen carefully and comply with all instruction given by the charity/instructors/helpers/marshals
