Terms and Conditions
Please note, funds raised for this appeal may be used for other, similar projects for the benefit of patients if our target is exceeded or not met, or if the appeal purposes cannot be fulfilled.
The Hayward House Memory Tree was designed and installed by artist Claudia Ashley-Brown.
Leaves are laser engraved brushed bronze, silver or gold effect, in return for an appropriate donation or subject to availability. Due to the limited number of leaves, any donation for a leaf is in addition to any funds already received. We are unable to back date donations to purchase a leaf.
Leaves will remain on the tree for one year, starting from the installation date. After this period, you will offered the opportunity to renew for a further 12 months. If you do not wish to renew your dedication, your leaf will be sent to you in a keepsake box.
The leaf will appear exactly as written – Nottingham Hospitals Charity is not responsible for mis-spelling.
Nottingham Hospitals Charity has the right to decline any donation or choice of wording for inscription, including promotional messaging. Nottingham Hospitals Charity reserves the right to move or relocate the Memory Tree to another appropriate site within the Hayward House premises.
